29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about October 18, 2017, Axe Roofing, LLC, as controlling employer, did not ensure that employees of All Home Reapirs Inc., subcontractor, were protected from overhead falling object hazards while working on the ground below roofing operations without wearing hard hats. A 2x4 with protruding nails was thrown from the roof in the area of a worker on the ground.
