29 CFR 1910.147(c)(7)(iii): The employer did not provide retraining for all authorized and affected employees whenever there was reason to believe that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures; and whenever needed to reestablish employee proficiency and introduce new or revised energy control methods and procedures: The Employer did not ensure effective training was provided and utilized on the energy control program and procedures to ensure employees obtained the knowledge and skills required. Deviations and inadequacies in the use of energy control exposed employees to being caught-in, struck-by and amputation hazards. a) on the Amanda Hydraulic Press Brake when changing out dies, and b) when bypassing the light curtains.
