29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating conditions. Entire facility- On or about October 23, 2017, the battery connector on a Toyota walkie rider (serial #47083) was defective, in that the outer insulation was pulled away and wires were exposed. Employees were exposed to the hazards associated with energized wires.
