29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.  (a) New residential construction located on Lot #125, Roof: On or about 10/24/2017, and at times prior, the employer exposed employees to fall hazards on an 8/12 pitched roof with eave heights up to 24-feet, in that adequate personal fall arrest systems were not provided. Anchor points, life-line attachments, lanyards, and body harnesses were not used according to the manufacturer's recommendations and the personal fall protection system requirements under CFR 1926.502(d).

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  New residential construction located on Lot #125, Roof: On or about 10/24/2017, and at times prior, the employer did not provide fall protection training to employees working on a 8/12 pitched roof at eave heights of 24-feet, 2-inches, in that employees were not trained to install a complete personal fall protection system with all of the components and proper attachment points recommended by the manufacturer. Training by a competent person was not provided b the employer as fall protection systems used were not adequate.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)  (a) New residential construction located on Lot #125, Front side: On or about 10/24/2017, and at times prior, the employer exposed employees to fall hazards of up to 10-feet, 2-inches while using an extension ladder to access the roof, in that the ladder had a damaged locking mechanism as the rivets holding the mechanism had broken off on the fly section of the ladder.
