29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) New residential construction located on Lot #64, Rear side: On or about 10/24/2017, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic nail guns at 120 PSI, in that protective eyewear was not provided to employees operating these tools while installing Hardie-Plank Siding.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) New residential construction located on Lot #64, Rear side: On or about 10/24/2017, and at times prior, the employer exposed employees to fall hazards of 10-feet, 8-inches to the lower level, in that a personal fall arrest system was nt provided to employees installing siding from a pump jack scaffold system.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) New residential construction located on Lot #64, Front side: On or about 10/24/2017, and at times prior, the employer failed to inspect all of the electrical cords and equipment prior to the start of the work shift, in that a 50-ft. extension cord used to power an air compressor had a damaged/torn outer casing which exposed the internal insulated wires and was not repaired or removed from service.
