29 CFR 1926.451(e)(1): When scaffold platforms were more than two (2) feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  At 1727 Lafayette Avenue, St. Louis, MO, the prefabricated ladder belonging to a tubular weld standard frame scaffold was not fully aligned.  The first three sections of ladder were aligned along the north end of each tier.  The fourth and highest section of ladder was positioned along the south end of the tier.  Employee(s) working from this four tier high scaffold were required to cross over from one end of the scaffold to the other when climbing up or down this ladder.  This violation was observed on 10/24/17.
