29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a) Facility Wide - On or about October 17, 2017, the employer did not ensure that each authorized employee was provided training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control when performing maintenance tasks, including checking size measurements of belts and filters and performing tune ups on HVAC units.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence to achieve energy control by performing the following steps as required by 29 CFR 1910.147(d)(1)-(6):  1.      Turn off and shut down power to the machinery,  2.      Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s),  3.      Affix lockout/tagout devices to each energy isolating device,  4.      Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work:      a) Second Floor Mechanical Room- On or about October 17, 2017, the employer did not ensure that the employee(s) performing maintenance activities on equipment including the HVAC unit, performed lockout utilizing the established procedure to de-energize the equipment to achieve energy control and eliminate residual energy.
