29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) 202 North 3 Notch St., Troy AL: On October 24, 2017 and at times prior to, the employer exposed employees to fall hazards by failing to ensure employees using a metal adjustable ladder to access flat roof 30 foot above a concrete surface extended the ladder 3 feet over the roof access.
