29 CFR 1910.133(a)(2): Each affected employee did not wear eye protection that provided side protection when there was a hazard from flying objects:   a)  On or about October 24, 2017, the employer did not ensure that employees were protected from eye injuries while operating the mills and lathes.  Employees were wearing side shields on their safety glasses that did not meet the ANSI Z87.1requirement.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a)  On or about October 24, 2017, the employer did not ensure that employees utilized energy control procedures on the Omni Turn CNC Lathes prior to conducting tool changes and job set up. The lathes were not isolated from the electrical energy source prior to engaging in these activities. Employees were exposed to hazards from the rotating parts while all hazardous energy sources were not controlled prior to conducting work.       As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, tool changes and setup operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  On or about October 24, 2017 the employer did not ensure the employees were protected from the rotating cutters on the Barker Mill.  Employees were exposed to amputation injuries while operating the machine due to the lack of guarding.   b)  On or about October 24, 2017 the employer did not ensure the employees were protected from the rotating cutters on the Cincinnati Mill.  Employees were exposed to amputation injuries while operating the machine due to the lack of guarding.   c)  On or about October 24, 2017 the employer did not ensure the employees were protected from the rotating parts on the Omni Turn Lathes (1&2).  Employees were exposed to amputation injuries while operating the machine due to the lack of interlocks on the doors.  d)  On or about October 24, 2017 the employer did not ensure the employees were protected from the rotating shaft on the Warner and Swasey No. 5 turret lathe.  Employees were exposed to caught by hazards while operating the machine due the lack of guarding.
