(a) On or about October 24, 2017, at 87-16 130th Street, Richmond Hill, NY 11418 An electrical box marked "vaccun" (next to box marked "green vacuum no. 3) located on the wall adjacent to the dust collectors had an uncovered hole, allowing combustible wood dust to infiltrate and accumulate inside the box, as well as potentially allowed contact with or damage to live wires. (b) On or about October 24, 2017, at 87-16 130th Street, Richmond Hill, NY 11418 A four outlet electrical box located on the wall near the dust collectors had an uncovered hole, allowing combustible wood dust to infiltrate and accumulate inside the box, as well as potentially allowed contact with or damage to live wires. (c) On or about October 24, 2017, at 87-16 130th Street, Richmond Hill, NY 11418 An electrical box attached behind a switch box marked "light" and "heat" and located on the wall adjacent to the dust collectors had an uncovered hole, allowing combustible wood dust to infiltrate and accumulate inside the box, as well as potentially allowed contact with or damage to live wires.
