29 CFR 1910.23(b)(2): Ladder rungs, steps, and cleats were not spaced less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, and steps: (a) On or about 10/17/2017 - Roof top HVAC access room, 15' metal fixed ladder, the distance from the top rung to the top of the metal edge of the hatch was 20 inches exposing employees to falls of approximately 16 feet.
