OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:         a) At the special needs pen, on or about 10/24/2017: Employees were exposed to fall        hazards of 10 feet to the lower level while storing and retrieving bedding for the cows, as        the area did not have guardrails.         b) At the special needs pen, on or about 10/24, 2017: Employees were exposed to fall        hazards of 10 feet to the lower level as the bedding storage area did not have safe        access or egress in that it lacked grab bars at the top of the access ladder.         c) At the milking parlor, on or about 10/24/2017: Employees were exposed to fall hazards       of 7 feet as the stairways, with approximately 12 steps, did not have hand rails.     Among other feasible and acceptable means of abatement: To abate the hazard, install a guardrail system and hand railing that will provide fall protection and will comply with the American National Standards Institute (ANSI) Standard A1264.1 - Safety Requirements for Workplace Walking/Working Surfaces and their Access, Section 5.1, which states that every open sided platform four feet or more above the ground level shall be guarded by a railing system, and Section 7.2, which states that every flight of stairs shall be equipped with a handrail system.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical hazards.          a) Throughout the farm, on or about 10/24/2018: Employees worked with injurious corrosives including but not limited to Ridstone, Mega San+, RTD CIP Detergent, Bac-Drop, and Classic Dynemate during cleaning, without facilities for quick drenching of the eyes within the work area for immediate emergency use in case of splashed chemical   onto the eyes.           b) Throughout the farm, on or about 10/24/2018:  Employees working with injurious corrosives including but not limited to Ridstone, Mega San+, RTD CIP Detergent, Bac-Drop, and Classic Dynemate during cleaning were not provided with chemical goggles and a faceshield to protect the eyes and face and chemical gloves and apron to protect the hands and body.         Among other feasible and acceptable means of abatement: Feasible means of Abatement:     Install eyewash facilities that will provide 15 minutes of continuous flow of water and will comply with the American National Standards Institute, ANSI Standard Z358.1-2014 - Eyewash Equipment, Section(s) 5.4 which states an eye wash station shall be located on the same level as the hazard, within 10 seconds to reach, with a highly visible sign and be connected to a supply of flushing fluid to produce spray for a minimum of 15 minutes.    Provide personal protective equipment, such as safety goggles, glasses, or face shield that will comply with Safety Data Sheets, Section 8, which states personal protection requires goggles, safety glasses or face shield for eye and face protection and protective gloves and clothing for skin and body protection.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fall hazards.         a) Throughout the farm, on or about 10/24/2018: The employer allowed employees to operate powered industrial machinery, such as but not limited to: skid steers, without the required training and certification to assure safe use and worker competency.          b) Throughout the farm, on or about 10/24/2017: An employee operated a skid steer, while another rode on the bucket, exposing the worker to struck-by and fall hazards of up to 6 feet.       Among other feasible and acceptable means of abatement: Provide training and certify workers to comply with manufacturer operator's manual and NIOSH Publication Number 2011-128, Preventing Injuries and Deaths from Skid-Steer Loaders, which states that workers must be trained to read and follow manufacturer's procedures for operating and maintaining skid-steer loaders [AEM 2016].

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck-by, caught-in, and amputation hazards.           a) At the maintenance shop, on or about 10/24/2018: Employees operated a Craftman table        saw without guard, spreader, or anti-kickback.         b) At the maintenance shop, on or about 10/24/2018: Employees operated a Porter Cable       bench grinder with a gap of ½ inch between the abrasive wheel and the work rest.       c) At the maintenance shop, on or about 10/24/2018: Employees operated a Duracraft         drill press without a guard around the rotating chuck.         Acceptable abatement methods to correct these hazardous conditions include compliance with:      a)    American National Standards Institute (ANSI) 01.1-2013 - Woodworking Safety Machinery Requirements, Section 5.2.1.2, which states that each saw shall be provided with a self-adjusting hood type guard that covers the blade above the workpiece, Section 5.2.1.4, which states each saw shall be furnished with a spreader, and Section 5.2.1.45 which states that an anti-kickback shall be provided.        b)    ANSI B7.1-1978 - Safety Requirements for the use, care, and protection of abrasive wheels, Section(s) 9.13, which states that the work rest shall be adjusted such that the gap between the work rest and the grinding face of the abrasive wheel shall not exceed 1/8 inch.        c)   ANSI B11.8-2001 - Safety Requirements for Manual Milling, Drilling and Boring Machines with or without         Automatic Control, per Figure A.4 or A.5, which indicate a guard, shall be installed around the rotating chuck.

Amongst other feasible means of abatement: To abate the hazards appropriately cover energized parts, and ensure strain relief at the cord end to comply with the National Electrical Code 70E, 2015, Article 245, which states that cover plates and cords shall be maintained, such that there are no missing cover plates, nor damage exposing live parts.

29 CFR 1928.57(b)(1)(i): All power take-off shafts, including rear, mid- or side-mounted shafts, shall be guarded either by a master shield, as provided in paragraph (b)(1)(ii) of this section, or by other protective guarding.        a) At the manure pump for the main barn, on or about 10/24/2018: A John Deer tractor was connected to the manure pump. The power take-off was not fully guarded against accidental contact.
