29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:    a) Big Shed - On or about October 24, 2017 and times prior to, the employer did not ensure that employees wore safety glasses over their prescription lenses or utilized eye protection that incorporated the prescription in its design.  Thus, employees who wore prescription lenses were exposed to eye injuries when using saws to cut lumber and using pneumatic nail guns to fabricate wooden trusses.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Big Shed - On or about October 24, 2017 and times prior to, the outer sheathing on the cables used to energize two electrical boxes for the motor on the 12-foot truss roller machine were pulled away from their connection points, thus exposing employees to electrical hazards.  b) Big Shed - On or about October 24, 2017, the outer sheathing on the cable used to energize the electrical disconnect box on the 14-foot truss roller machine was pulled away from its connection point at the box, thus exposing employees to electrical hazards.
