(2) Foot Brake coasting during stopping application. (3) No Safety Locking Pin on left side fork blade connector.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours: (a) On October 18, 2017, the employer did not report the in-patient hospitalization of an employee within the twenty-four hour requirement. An employee received a struck-by injury while working in the Grinding Area of the facility located at 101 Industrial Dr., Monett, MO., was taken and admitted to a local hospital for medical treatment. The event was not reported to OSHA by the employer.
