29 CFR 1910.23(c)(4): Unsecured ladder(s) were used on surfaces that were not stable and level:    On or about October 24, 2017, at the above addressed jobsite, an employee performing tree work was exposed to a fall hazard of approximately 18 to 22 feet when using an extension ladder to gain access to and from a tree while the ladder was not secured to prevent accidental displacement.

29 CFR 1910.28(b)(15)(iii): Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level is protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems:    On or about October 24, 2017, at the above addressed jobsite, an employee performing tree work was exposed to a fall hazard of approximately 22 feet when standing on previously cut tree limbs and branches while not being protected from falling to the lower level.

29 CFR 1910.30(a)(3)(iv): The employer did not train each employee on the correct use of personal fall protection systems and equipment, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.   On or about October 24, 2017, at the above addressed jobsite, an employee exposed to a fall hazard while performing tree work was not provided with a training program that would enable him to recognize the hazards associated with falls and the correct use of personal fall protection systems and equipment.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  On or about October 24, 2017, at the above addressed jobsite, employees were using chemicals including but not limited to Gasoline, Husqvarna Bar & Chain Oil and Stihl 2-Cycle Engine Oil to power a chain saw and pole saw while the employer did not maintain the safety data sheets for that products at the worksite.
