29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Excavation Site: On or about October 25, 2017; The employer did not determine by a safe and acceptable means the exact location of the gas line. The gas line was struck by the backhoe bucket being used to dig out the excavation. This condition exposed the employees to entrapment, cave-in and or explosion hazards.
