29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) On or about November 8, 2017 and at times prior, employees were required to use a bench grinder with the work rest opening of approximately one inch. The work rest of the bench grinder should have a maximum opening of one eight inch distance from the abrasive wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) On or about November 8, 2017 and at times prior, employees were required to use a bench grinder with the tongue guard opening of approximately one inch. The tongue guard of the bench grinder should not exceed a one fourth inch distance from the abrasive wheel.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): a) On or about November 8, 2017 and at times prior, employees were exposed to possible health illness while handling hazardous chemicals in the workplace. Several containers in the workplace were not labeled with the identity of the contents and an appropriate hazard warning: 1. PB Blaster liquid- contains Petroleum distillates which can cause nausea; dry cracked skin; and chemical pneumonitis (aspiration liquid). 2. Platinum antifreeze fluid - contains ethylene glycol which can cause poisoning or death. 3. Locomotive heavy duty - contains Potassium Hydroxide which can cause eye burns, ingestion, and inhalation problems.
