Lyman Terrace) Holyoke, MA 01040 on 10/26/2017, and at times earlier, employees engaged in window blocking and shear wall installation were working at a height greater than 6 feet and were exposed to floor and wall holes which were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employees provided with alternative fall protection measures.
