Paramount Games, Inc. - On or about 10/26/17, and at times prior thereto, employees were performing servicing and maintenance on equipment including, but not limited to, the banded ticket machines. The servicing and maintenance included, but was not limited to, replacing motors and preventative maintenance. The company did not have lockout/tagout procedures for the machinery.

Paramount Games, Inc. - On or about 10/26/17, and at times prior thereto, employees were performing servicing and maintenance on equipment including, but not limited to, the banded ticket machines. The servicing and maintenance included, but was not limited to, replacing motors and preventative maintenance. The company did not perform periodic inspections of the lockout/tagout procedures with authorized employees.
