29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: JOB SITE: 1230 Culverhill Drive, Webster Groves, MO 63119 on or about 10/25/2017, at this residential structure, workers were observed on top of the roof, actively installing new roof shingles, operating electrical power hand tool without any form of eye or face protection. The workers were exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: JOB SITE: 1230 Culverhill Drive, Webster Groves, MO 63119, on or about 10/25/2017, workers were observed on top of a 6:12 pitch roof, approximately 10' 8" eave to ground level, actively installing black felt paper and removing roof vents, and were not protected by guardrail systems, safety net system, or personal fall arrest system. Although the workers were wearing Personal Fall Arrest Systems (harnesses), but none of the workers were tied off to an anchor point with lanyard. The workers were exposed to a fall hazard
