29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as the opening of process equipment or piping of a covered process: a) On or about October 18th 2017, in the C-line unit at the Braskem facility located at La Porte Texas, the employer did not develop and implement safe work practices to provide for the control of hazards during the opening of a high pressure bag filter including but not limited to the: I) deactivation of the steam trace before opening the high pressure bag filter II) Introduction of water to knock out polymer fines from the high pressure bag filter. This condition exposed employees to high temperature water hazards

29 CFR 1910.119(j)(2): The employer did not implement an adequate written procedure to maintain the on-going integrity of process equipment (a) On or about October 18, 2017, the written procedure for changing the filter on the high pressure bag filter separator in the C-line unit did not adequately address how to safely add and remove water to the inside of the vessel so that employees would not be exposed to the risk of coming in contact with hot water.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the equipment were not physically located and operated in such a manner as to isolate the machine equipment from the energy source: (a) On or about October 18th 2017, in the C-line unit at the Braskem facility located in La Porte Texas; the employer failed to ensure that the isolating devices needed to control the hazardous energy for the high pressure bag filter (F-300-C) were applied in a manner so as to isolate the equipment from the steam trace. This condition exposed employees to high temperature water hazards.
