29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about 10/25/2017, St. Louis Steel Construction, LLC was working on a commercial building at 100 Quartz Canyon Drive in Wentzville, MO. An employee was installing decking around the perimeter of the roof while working from a Skyjack Aerial Lift, Model # SJ45T. The employee was not wearing a body belt with lanyard attached to the boom or basket while working from the lift.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about 10/25/2017, St. Louis Steel Construction, LLC was working on a commercial building at 100 Quartz Canyon Drive in Wentzville, MO. At the time of the inspection, an employee was installing decking around the perimeter of the roof and was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.
