29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Work site - On or about October 26, 2017 and at times prior to, sub-contractors employees were working from a scaffold 3 sections high, that had only two scaffold planks installed with one section only having one plank.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) Work site - On or about October 26, 2017 and at times prior to, employees erected a 3 section high, 4 section wide scaffold along side energized electrical lines and did not fully plank the working surfaces. A competent person would have recognized these hazards and stopped work.
