29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k).  On or about November 21, 2017, the employer did not train each employee on the effects of noise on hearing; the purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, instructions on selection, fitting, use, and care; the purpose of audiometric testing, and an explanation of the test procedures. Employees were exposed to noise above the action limit, including:  a) Employee one was exposed to continuous noise levels at 103.7-percent of the permissible daily noise exposure, or an equivalent sound level of 90.3-dBA, during the 421-minute sampling period on November 21, 2017; exposure calculations include a zero increment of 59-minutes not sampled.  b) Employee two was exposed to continuous noise levels at 125.5-percent of the permissible daily noise exposure, or an equivalent sound level of 91.6-dBA, during the 396-minute sampling period on November 21, 2017; exposure calculations include a zero increment of 84-minutes not sampled.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about October 26, 2017, employees in the Pour Room were exposed to electrical shock and burn hazards from flexible cords used in lieu of fixed wiring, including:  a) a black flexible cord in series between a wall mounted switch and wall mounted receptacle;  b) an orange flexible cord used to power the concrete mixer;  c) an orange flexible cord used to power a vibration table; and  d) a black flexible cord used to power a vibration table.
