Hillz Excavating, Inc. on or about October 26, 2017, at the site located in the intersection of Scott Hamilton Ave. and Troup St. Bowling Green, OH: The employer did not ensure an employee was protected from head injuries while working in an excavation below overhead hazards such as undermined pavement and excavation equipment. The employee was exposed to potential impact injuries associated with falling equipment and/or debris.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: (a) B. Hillz Excavating, Inc. on or about October 26, 2017, at the site located in the intersection of Scott Hamilton Ave. and Troup St. Bowling Green, OH: The employer did not ensure undermined pavement surrounding the excavation in which employees were working was removed or adequately supported. The employees performing work activities this condition were exposed to struck-by hazards in the event of structure collapse or failure.
