OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or caught-in hazards:  (a) Assembly Area - The foot pedal, for actuating the horizontal air press did not have protection to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedal, exposing employees to the hazard of getting hands and fingers struck by or caught in the press, on or about October 26, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Teledyne SSI, 349 Science Park Drive, State College, Pennsylvania - Employees performed servicing and maintenance activities, such as mold changing, on a variety of machinery and equipment, including, but not limited to, the Cincinnati Milacron Vista Sentry plastic injection molding machine, and the employer did not develop and implement machine specific energy control procedures, exposing employees to the hazards of getting caught-in and struck-by moving machine parts, on or about October 26, 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Machine Shop, near Injection Molding Room - The homemade tubing sander, used to lap the ends of small metal tubes, had no guarding around the wheel/plate holding the abrasive sanding disk, exposing employees to rotating parts and entanglement hazards, on or about October 26, 2017.    Abatement certification required within 10 days after abatement date. 29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    (a) Injection Molding Room - An employee used a compressed air nozzle to clean at the Cincinnati Milacron injection molder.  The compressed air nozzle had a tip pressure measured at 110 p.s.i., exposing the employee to the hazard of skin penetration with compressed air, on or about October 26, 2017.    (b) Shop - An employee used a compressed air nozzle to blow down the Hardinge Conquest T42 CNC machine.  The compressed air nozzle had a tip pressure measured at 100 p.s.i., exposing the employee to the hazard of skin penetration with compressed air, on or about October 26, 2017.    (c) Shop - An employee used a compressed air nozzle to blow down the Hardinge CNC Lathe Twin Turn 65.  The compressed air nozzle had a tip pressure measured at 100 p.s.i., exposing the employee to the hazard of skin penetration with compressed air, on or about October 26, 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Machine Shop, near Injection Molding Room - The employer used a Woods relocatable power tap (a.k.a., a power strip or surge protector) to power loads, including a small shop vac, the homemade tubing sander, and the Carboloy grinder.  The UL listing for the power tap specifies use in supplying power to low amperage electronic equipment rather than equipment such as the shop vac, tubing sander, and grinder, exposing employees to an electrical hazard, on or about October 26, 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.
