29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the residential construction site located at 1134 N Euclid Ave in Kansas City, Missouri: Employees engaged in roofing activities were exposed to injury from a 13 foot fall hazard while accessing and exiting the roof. The ladder that provided access to the roof on the front of the house did not extend 3 feet above the eave.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the residential construction site located at 1134 N Euclid Ave in Kansas City, Missouri: Employees engaged in residential roofing activities from a 8:12 pitch roof were exposed to fall hazards of approximately 16 feet from the eve of the residence to the ground below in that fall protection was not utilized. Raul Lera Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 126.501(b)(11), which was contained in OSHA inspection number1251954, citation number1, item number 2 and was affirmed as a final order on 10/6/2017, with respect to a workplace located at 8622 Blue Ridge Blvd in Kansas City, MO.
