29 CFR 1910.23(d)(4): The side rails of through or side-step ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder.      On or about October 11, 2017 at the Sludge Dryer, employees that access the upper level to check the buildup on the paddles in the sludge dryer were exposed to a fall hazard of approximately 7'-2" to the lower level when there was only one side rail provided on the fixed ladder.

29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.    a. On or about October 11, 2017 employees were exposed to fall hazard of more than 4 feet to the lower level when working at the following location:      When working at  Racks #3, #5, and #6, employees have to clean the fly bars on the racks, retrieve racks that have fallen off the hooks into the tanks by climbing on top of the tanks.

On or about October 11, 2017 Racks #5 and #6, employees required to move the carts between tanks, clean the bars on the racks, or retrieve racks that have fallen into the tanks were exposed to a fall hazard of approximately 52" to the lower level when the midrails were missing on the platform.
