29 CFR 1926.651(c)(2):  A stairway, ladder, ramp, or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:      The employer did not ensure that a safe means of egress, such as a stairway, ladder or ramp was provided for employees of its sub contractor, Bryant Construction Company, Inc., that were working in a excavation that was approximately 13.4 feet in depth.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:      Employees of its sub contractor, Bryant Construction Company, Inc., were welding a cap on a water line in the north portion of an excavation at a depth of 13.4 feet.  Standing water was present and protective measures and/or water removal methods were not used in this portion of the excavation.

29 CFR 1926.651(k)(1):  An inspection of the excavation, the adjacent areas, and protective systems was not conducted by a competent person prior to the start of work and as needed throughout the shift:      The employer did not ensure the inspections were conducted by a competent person prior to start of work to identify hazards associated with working in an excavation.  An employee of its sub contractor, Bryant Construction Company, Inc., was exposed to cave-in hazards when working in an unprotected excavation approximately 13.4 feet in depth, approximately 14 feet wide, approximately 24 feet in length.
