29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): a) On 10/28/2017 at 187-10 Jamaica Ave. Hollis, NY 11423 While operating a 960F wheel loader to bring cardboard back from a truck to inside an Outbound Corrugated Cardboard (OCC) area. The wheel loader was backed into bundles of cardboard by the operator causing the building wall to collapse onto two employees who were working outside. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
