29 CFR 1910.147(c)(7)(iii)(B): Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: (a) On or about October 29, 2017 - Rehang Area, employees who clean, service, and maintain the Meyn Automatic Rehang Machines between the Kill Line and Evisceration Lines 1 and 2 had not received training on where to apply the three locks on the electrical breakers in order to ensure energy isolation.

OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in and struck-by hazards: (a) On or about October 29, 2017 - Rehang Area, the guarding was not adequate on the Meyn Rehanger Machines to protect employees from the rotating parts and ingoing nip points of the shackles and equipment, to include but not limited to the sunflower wheel. Among other methods, one feasible and acceptable method to correct these hazards would be to ensure that proper guarding is utilized to protect employees working in close vicinity to the moving equipment as addressed in American Society of Mechanical Engineers (ASME) B20.1-2003 "Safety Standard for Conveyors and Related Equipment" section 5.9.1.2 Interfacing of Equipment that states "when two or more pieces of equipment are interfaced, special attention shall be given to the interfaced area to ensure the presence of adequate guarding and safety devices.
