The employer did not provide training to the authorized employee on lockout/tagout.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Al Fab Area - On or about October 27, 2017, the employer did not control the hazardous energy of the "CTD" Chop Saw exposing an employee to struck-by and caught-in hazards of the rotating 16 inch saw blade. Trojan Leisure Products, LLC DBA Radiant Pools was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 942512, citation number 2, item number 2b, and was affirmed as a final order on 3/28/14, with respect to a workplace located at 440 North Pearl Street, Albany, NY 12207.
