OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working underneath a suspended pedestrian bridge were exposed to a crushing hazard: a) On or about September 27, 2017, three employees were walking with an inspector underneath a pedestrian bridge 10 feet wide and 60 feet long, supported by two metal sawhorses. The employees were looking at welds that were in need of repair based on the inspector's recommendation. While they were under the pedestrian bridge one sawhorse failed, crushing the two employees underneath the pedestrian bridge, which resulted in a broken leg and head injuries.
