29 CFR 1904.39(a)(2): An oral or written report of a work related incident which resulted in an in-patient hospitalization of one or more employees was not reported to the Occupational Safety & Health Administration (OSHA) within 24 hours after the occurrence: a- GMP Contracting LLC, 2240 South Clinton Ave, South Plainfield, NJ 07080 - Throughout Facility & Worksites -Notification of a work related incident which occurred on 8/18/2017 which resulted in a hospitalization was not made by the employer to OSHA within 24 hours after having knowledge of the occurrence on 9/14/2017. Condition noted on or about 11/1/17.
