This condition exposed employee to caught-in-between and stuck-by hazards associated with the improper operation of the forklifts.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)Easi-Pour, LLC at 550 Nevada Avenue Southwest, Huron, SD 57350: On and prior to November 6, 2017, the employer did not ensure employees removing roof panels on the south end of the roof of the old shop, which was located within the new addition to the shop, were protected by a fall protection system. On October 31, 2017, two employees fell while attempting to remove roof panels.  An employee working on the roof engaged in removing the roof panels fell approximately 14-feet and landed on top of the overhead crane. Another employee fell approximately 21-feet and died due to injuries sustained from the fall. The condition exposed employees to falls of approximately 14 and 21-feet.
