29 CFR 1910.132(f)(3): When the employer had reason to believe that an affected employee who had already been trained did not have the understanding and skill required by paragraph (f)(2) of this section, the employer did not retrain that employee. In the front plate dry cast, employees engaged in setting metal rings without the use of leather heavy duty gloves were exposed to caught-in, and amputation hazards. The employer did not provide retraining on proper protective equipment.

29 CFR 1910.138(a): Appropriate hand protection was not used when employee's hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. In the front plate dry cast, the employer did not ensure employees used the proper gloves while moving heavy round metal plates exposing employees to caught-in and struck-by hazards.
