29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in, and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 10/24/17, the employer failed ensure the following steps were taken when employees were conducting cleaning activities on machinery, including, but not limited to Blender #2. Energy sources including, but not limited to electrical and mechanical. (Note: all steps are to be done by an authorized employee(s)): 1) Prepare for shutdown, 2) Shutdown machine or equipment, 3) Machine/equipment isolation, 4) Apply lockout device(s), 5) Relieve potentially hazardous energy sources, and 6) Verify energy isolation.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about 10/24/17, the employer failed to provide adequate lockout/tagout training to authorized employees. Employees were entering machinery, including, but not limited to Blender #2 without understanding lockout requirements.The employees did not understand the definitions of affected and authorized employees, and were not aware of what qualified as service and/or maintenance.
