29 CFR 1926.1204(b): The employer had not identified and evaluated all the hazards of permit required confined spaces before employee entry: a) On or about October 25, 2017 at 18:00 hours at a manhole near the intersection of Semmes Road and Pershing Road, Fort Jackson, S.C.: The employer had only identified "atmosphere" as a hazard in this permit required confined space, and did not address the hazards of pressurized hot water and electrical tools when introduced into the space. An accident occurred when an employee using a hand-held electrical reciprocating saw cut into a pressurize sewer liner containing hot water causing the water to spew upward upon the employee causing 2nd degree burns.
