On or about October 27, 2017 at 86-57 Midland Parkway, Jamaica Estates, NY 11432 The cover of a floor hole located on the residential construction project was observed not secured exposing employees to a fall hazard of approximately eight (8) feet to the lower level. ABATEMENT CERTIFICATION IS NOT REQUIRED
