29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a) Corestates Processing & Recycling: An employee, Shredder/Grinder Operator, was exposed to continuous noise at 78.5% of the exposure level of 90 dBA or an 8-hour time-weighted average of 88.3 dBA. This exposure was observed over a 207 minute sampling period. Zero exposure was assumed for the 273 minutes not sampled. The employer did not develop/implement a noise monitoring program for those employees exposed over the action level of 85 dBA, as discovered on or about 11/07/2017. ABATED DURING INSPECTION NO ABATEMENT CERTIFICATION REQUIRED

This exposure was observed over a 207 minute sampling period. Zero exposure was assumed for the 273 minutes not sampled. The employer did not establish/maintain a noise training program for those employees exposed over the action level of 85 dBA, as discovered on or about 11/07/2017. ABATEMENT CERTIFICATION REQUIRED ABATEMENT NOTE Annual noise training shall include the following: 1. The effects of noise on hearing; 2. The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care; and 3. The purpose of audiometric testing, and an explanation of the test procedures.

Signs to indicate the nearest exits were not posted in these aisles when the visibility of exit discharges was obstructed by pallets of stored materials, as discovered on or about 11/01/2017.
