29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Association Construction Roofing, LLC at 4021 Umatilla St. Denver, CO 80211: On or before 11/1/2017, the exposing, creating and controlling employer did not protect employees from injury in that hard hats were not provided or required for employees working beneath other employees who were tearing off an old wood-shingled roof. Wood shingles, tar paper and nails were being tossed from the roof directly onto and around the employee working beneath. This condition exposed employees to falling objects.  Association Construction Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 894543, citation number 2 , item number 1 and was affirmed as a final order on 4/17/2013, with respect to a workplace located at 5941 S. Eudora Way Centennial, CO 80121.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Association Construction Roofing, LLC at 4021 Umatilla St. Denver, CO 80211: On or before 11/1/2017, the exposing, creating and controlling employer did not protect employees from injury in that four employees working on a 9-in-12 steep-pitched roof were not wearing fall protection. This condition exposed employees to a fall hazard of 12 feet.  Association Construction Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1158341, citation number 2, item number 1 and was affirmed as a final order on 8/22/2016, with respect to a workplace located at 2773 White Oak St. Highlands Ranch, CO 80129.  Association Construction Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 894543, citation number 2, item number 2 and was affirmed as a final order on 4/17/2013, with respect to a workplace located at 5941 Eudora St. Centennial, CO 80121.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Association Construction Roofing, LLC at 4021 Umatilla St. Denver, CO 80211: On or before 11/1/2017 the exposing, creating and controlling employer did not protect employees from injury in that the aluminum extension ladder used by four employees to access the roof did not extend at least three feet from the roof's eave. This condition exposed employees to a fall hazard of 12 feet.  Association Construction Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1158341 citation number 1 item number 1 and was affirmed as a final order on 8/22/2016, with respect to a workplace located at 2773 White Oak St. Highlands Ranch, CO 80129.
