29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Association Construction Roofing, LLC at 4021 Umatilla St. Denver, CO 80211: On or before 11/1/2017, the exposing, creating and controlling employer did not protect employees from injury in that hard hats were not provided or required for employees working beneath other employees who were tearing off an old wood-shingled roof. Wood shingles, tar paper and nails were being tossed from the roof directly onto and around the employee working beneath. This condition exposed employees to falling objects.  Association Construction Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 894543, citation number 2 , item number 1 and was affirmed as a final order on 4/17/2013, with respect to a workplace located at 5941 S. Eudora Way Centennial, CO 80121.
