29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) In the containment rooms - The purpose built "D" machines were not adequately guarded to protect employees from reaching into various moving parts of the machine while loading materials into the hoppers. On or about October 20, 2017, an employee operated one of the "D" machines to produce powder filled projectiles. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) The documented energy control procedures were inadequate in that they did not include information such as the scope of the procedure, the purpose of the procedure, the magnitude of the energy source(s) expected to be encountered and the means to enforce compliance with the procedure(s). On November 1, 2017 a horizontal injection molding machine was observed in a "locked out" condition with a tag that indicated the machine was locked out on or about October 9, 2017. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
