29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) Jobsite, Area of 418 California Avenue, Oakmont, PA 15139: On or before November 1, 2017, an employee was working within a 4 foot 5 inch trench without a ladder for safe access and egress.

29 CFR 1926.651(g)(1)(i): Where oxygen deficiency (atmosphere containing less than 19.5 percent oxygen) or a hazardous atmosphere existed or could reasonably be expected to exist, such as in excavations in landfill areas or excavations in areas where hazardous substances are stored nearby, the atmospheres in the excavation shall be tested before employees enter excavations greater than 4 feet (1.22 m) in depth:  a) Jobsite, Area of 418 California Avenue, Oakmont, PA 15139: On or before November 1, 2017, an employee was permitted after a natural gas leak to enter an adjacent trench and install a pipe plug prior to the employer ensuring the trench was free of a hazardous atmosphere.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a) Jobsite, Area of 418 California Avenue, Oakmont, PA 15139: On or before November 1, 2017, the competent person did not take all necessary precautions to ensure employees were protected while accessing and working within the trench.
