29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Broadstone Westside Apartments, Fourth level of framing: On or about 11/01/2017, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that protective eyewear was not provided while erecting guardrails on the edges of the walking/working surface.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Broadstone Westside Apartments, Fourth level of framing (6th floor): On or about 11/01/2017, and at times prior, the employer exposed employees to fall hazards of approximately 31-feet, 9-inches to the lower ground level, in that employees erecting guardrails at the edge of the walking/working surface were not provided with a fall protection system. (b) Broadstone Westside Apartments, Fourth level of framing(6th floor): On or about 11/01/2017, and at times prior, the employer exposed employees to fall hazards of approximately 31-feet, 9-inches and greater to the lower ground level, in that guardrails were not installed along all open ends of the walking/working surface.
