29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a) Production Area: The employer did not provide a lock to every authorized machine adjuster. The authorized machine adjusters are required to perform tasks that require lockout/tagout as part of their regular job duties. These tasks include, but are not limited to, clearing paper jams and making machine adjustments. On or about August 28, 2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Production Area: The employer did not ensure that all employees utilized lockout/tagout procedures when removing jams from the Peerless Plate machines. An employee removed a paper jam from lane 2 on machine #23 without implementing any energy control procedures. On or about August 28, 2017. b) Production Area: The employer did not ensure that all employees utilized lockout/tagout procedures when removing jams from the Peerless Plate machines. An employee removed a paper jam in the in-feed area of the Peerless Plate machine # 25, near the cutting head and in feed roller, while the machine was powered. The employee did not implement any energy control procedures. On or about August 28, 2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. The AJM Packaging Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1030948 on citation number 1 and item number 1 and was affirmed as a final order on 8/11/15, with respect to a workplace located at 3401 South East Boulevard Vineland NJ, 08360.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Production Area: The area located below the interlocked gates in the back of the Peerless plate machines was not guarded. This area allows access to internal machine components including, but not limited to, the rails, the forming dies, and the cutting dies, and contains pinch points and moving parts. Employees regularly perform work in close proximity to the unguarded area while the machine is in operation, and are also required to access the area to perform tasks including, but not limited to, removing paper jams and making machine adjustments. On or about August 28, 2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
