29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Facility Wide: On or about November 3, 2017 and times prior, employees performed servicing and maintenance on equipment such as a Marvel Vertical Bandsaw that operated on 460 volts.  A program consisting of energy control procedures, employee training and periodic inspections was not established.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:    a) Facility Wide: On or about November 3, 2017 and times prior, employees working  with equipment such as bandsaw, power presses, shears, welding and lathes, without an infirmary, clinic or hospital in near proximity to the workplace, were not provided with a person onsite who is trained in first aid.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) River Shop area: On or about November 2, 2017 and times prior, employees used a Marvel vertical bandsaw to cut metal sheets to size.  The  guide was not properly adjusted and 28 1/2 inches of the blade was left unguarded.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a)River Shop Vertical Band Saw:  On or about November 2, 2017 and times prior a 15 foot extension cord provided power to  a water pump on a Marvel Vertical Bandsaw.  The extension cord was being used in lieu of permanent wiring.
