29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: Warehouse: On or about October 31, 2017, wheel chocks placed under the truck's rear wheels were removed while the truck was boarded with the powered industrial truck. Power industrial truck fell to the ground. This condition exposed employee struck-by hazards.
