29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.  (a)  1501 Willow Street, Lebanon Pennsylvania, West Unit  -  The employer did not ensure that the all  piping on Compressor #8 met the labeling requirements that comply with recognized and generally accepted good engineering practices, on or about November 3, 2017.  (b) 1501 Willow Street, Lebanon Pennsylvania, West Unit - The employer did not ensure that all the piping on the Pump Package met the labeling requirements that comply with the recognized and generally accepted good engineering practices, on or about November 3, 2017.   (c) 1501 Willow Street, Lebanon Pennsylvania, West Unit  - Employees working in the west engine room were not provided an inside emergency shower as required by recognized and generally accepted good engineering practices, on or about November 3, 2017.  (d)  1501 Willow Street, Lebanon Pennsylvania, West Unit  - Employees working in the west engine room were not provided an outside emergency eye wash and shower within the required distance, that meet the requirements of recognized and generally accepted good engineering practices, on or about November 3, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the team's findings and recommendations; assure that the recommendations were resolved in a timely manner and that the resolution was documented; document what actions were to be taken; complete actions as soon as possible; develop a written schedule of when these actions were to be completed; communicate the actions to operating, maintenance and other employees whose work assignments were in the process and who may have been affected by the recommendations or actions:  (a)  1501 Willow Street, Lebanon Pennsylvania, West Unit  - The employer did not resolve the finding of the PHA in a timely manner, on or about November 3, 2017.
