29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A. On or about November 8, 2017: Two Atom S 530 CN CNC die-cutting presses were not effectively guarded to protect employees from contact with pinch points created by the movement of the flat clamps. The clear guard at the front of the equipment left an opening large enough to allow access to moving parts.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: A. Applied Rubber and Plastics, Inc., 100 Skitchewaug Street, Windsor, Connecticut: On or about November 8, 2017, the employer did not record the following workplace injury on the OSHA Form 300 or equivalent for the calendar year 2017: 1. On or about November 1, 2017, an employee sustained a fingertip amputation injury when caught in an unguarded pinch point on the Atom S 530 CN CNC die-cutting machine. The injury resulted in days away from work as well as restricted duty. No OSHA 300 log was maintained. The injury would have been properly recorded with a check mark in Column H (days away from work), the length of time away in Column K (number of days away), the length of time on restricted duty in Column L (number of days restricted/transferred) and a check mark in Column M1 (injury). Abatement Certification and Documentation is required
