29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) On or about 11/03/17, in the facility at 84-02 72nd. Glendale, NY A powered industrial truck operator was allowed to operate a forklift at the site without having received formal instruction, including operator evaluation to determine the retention of the information. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
